Mainly for adding support for VFIO DMABUF. While at it, update all
headers.
The header update breaks virtio-net due to virtio_net_hdr_v1_hash
changes. Include the virtio-net changes to avoid build and bisect
failures.

This patches enable the Address Translation Service support for virtio
pci devices. This is needed for a guest visible Device IOTLB
implementation and will be required by vhost device IOTLB API
implementation for intel IOMMU.

It's being used by the hypervisor. For now simply mimic a device not
capable of masking, and fully emulate any accesses a guest may issue
nevertheless as simple reads/writes without side effects.

Many of these should be cleaned up with proper qdev-/QOM-ification.
Right now there are many catch-all headers in include/hw/ARCH depending
on cpu.h, and this makes it necessary to compile these files per-target.
However, fixing this does not belong in these patches.

Implement an I/O space index-data register pair as defined by the AHCI
spec, including the corresponding SATA PCI capability and BAR.
This allows real-mode code to access the AHCI registers; real-mode
code cannot address the memory-mapped register space because it is
beyond the first megabyte.
